Current Returning Crawford Student and Incoming SDUSD Student Transfer Enrollment for 2025-2026

Option 1: Email Submission Enrollment

• 1. PreK-Grade 12 Enrollment Forms

     English | Spanish

  2. Universal Form

     English  | Spanish | Somali

  3. Health Information Exchange

     English | Spanish

• Fill out downloaded documents and save as PDF documents on computer. Please be sure to complete all pages.
• Email documents along with proof of address (SDG&E, Cable, Lease Agreement, etc.) to:

• Students will receive the schedule on the first day of SY 2025-2026.

   

   Option 2: In-Person Enrollment

• Enrollment Packet will be available for student/parents at school site during scheduled walk-in enrollment.
• Enrollment schedule is as follows:

9:00AM to 2:00PM Weekdays until further notice.
